Yes. You must report each employee’s:

9-digit Social Security number

Resident address (no PO Boxes) and resident PSD Code

Workplace location address (no PO Boxes) and non-resident PSD Code

The payroll location address or corporate address (if different from the employee’s workplace location)

Quarterly earnings

Local earned income tax withheld (based on resident and non-resident tax rates)
